Before we can process this referral we need the following information:

1. Patient demographic sheet
2. A signed order with diagnosis and reason/indication for exam
3. Licensed provider signature, either handwritten or electronically signed

For OB ultrasound please call 801-581-7746 to schedule an appointment and fax referral to 801-213-6946.
